Description

Norvir contains an active ingredient called Ritonavir which is a peptidomimetic inhibitor of the HIV-1 protease.

Indications:

- treatment of HIV type 1 infection in combination with other anti-retroviral agents.

MOA (mechanism of action):

Ritonavir is a protease inhibitor that binds to the protease active site and inhibits the activity of the enzyme. This averts cleavage of the viral polyproteins resulting in the formation of immature non-infectious viral particles. Ritonavir is also active against HIV-2 protease.

Dosage

For adults:

When used as a booster for other protease inhibitors: 100 to 400 mg/day orally in 1 or 2 divided doses.

When used solely as an anti-retroviral agent:

Initial dose: 300 mg orally twice a day; increase by 100 mg twice a day every 2 to 3 days to the full maintenance dose.

Maintenance dose: 600 mg orally twice a day.

For children (older than 1 month):

When used solely as an anti-retroviral agent:

Initial dose: 250 mg/m2 orally twice a day; increase by 50 mg/m2 twice a day every 2 to 3 days to the full maintenance dose.

Maintenance dose: 350 to 400 mg/m2 orally twice a day.

Maximum dose: 600 mg/dose.

Precautions:

HIV can be transmitted through sexual contact with HIV positive partner, blood (e.g., sharing needles, handling blood instruments, receiving blood transfusions, blood products, or organ/tissue transplants that were infected with HIV), childbirth, toiletries (razors and toothbrushes). Therefore, both HIV-positive and HIV-negative individuals are advised to have any kind of sex using a latex or polyurethane condom, avoid sharing needles or other things like toothbrushes or razors. It is important to discuss with doctor the precautions needed to avoid infecting other people as well as take precautions not to get infected.

Women who are HIV-positive must not breastfeed because HIV infection can be passed on to the baby in breast milk.

Some people who take Norvir may experience:

- Liver problems. Some people taking Norvir in combination with other anti-HIV drugs have developed liver problems. Contact your healthcare provider immediately if the signs of liver problems occur.

- Changes in the electrical activity of the heart called PR prolongation. PR prolongation can cause irregular heartbeats.

- Immune reactivation syndrome (severe immune deficiency during combination anti-retroviral therapy (CART) may lead to arousal of opportunistic infections and cause serious clinical conditions).

- Increase in cholesterol and triglyceride levels.

- Diabetes and high blood sugar (hyperglycemia). Tell your healthcare provider if you notice an increase in thirst or frequent urination.

- Change in body fat (fat accumulates in the upper back and neck (“buffalo hump”), breast, and around the middle part of your body (trunk).

- Increased bleeding for hemophiliacs.

Missing a dose

If you take Norvir once daily: if you notice within 12 hours of the time you usually take Norvir, you must take the tablet(s) as soon as possible. Always take the tablet with food and with a dose of your other protease inhibitor. Then take the next dose as usual. If you notice 12 hours or more after the time you usually take Norvir, then do not take the missed dose. Wait and take the next dose, with food, at your usual time. Do not take a double dose to make up for missing one.

If you take Norvir twice daily: if you notice within 6 hours of the time you usually take Norvir, you must take the tablet as soon as possible. Always take the tablet with food and with a dose of your other protease inhibitor. Then take the next dose as usual. If you notice 6 hours or more after the time you usually take Norvir, then do not take the missed dose. Wait and take the next dose, with food, at your usual time. Do not take a double dose to make up for missing one.

Overdose

If an overdose occurs and you are not feeling well, stop taking the medication immediately. You should seek emergency medical attention or contact your healthcare provider immediately.

Storage

Store Norvir tablets or oral powder at room temperature between 68-77 degrees F (20-25 degrees C). Keep Norvir in the original container. Do not use Norvir if the seal over the bottle opening is broken or missing. Keep the bottle tightly closed. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

Side effects

The most common side effects associated with Norvir are:

  • diarrhea
  • nausea
  • vomiting
  • upper and lower stomach (abdominal) pain
  • tingling feeling or numbness in hands or feet or around the lips
  • rash
  • feeling weak or tired

Side effects occurrence does not only depend on the medication you are taking, but also on your overall health and other factors.

Contraindications

Norvir is contraindicated in patients who are allergic to Ritonavir or any of the other ingredients of Norvir.

Do not take Norvir if you have kidney, liver problems, hepatitis B or C and, hemophilia, diabetes mellitus, erectile dysfunction.

Do not combine Norvir with such medications as astemizole or terfenadine (for allergy); amiodarone, bepridil, dronedarone, encainide, flecainide, propafenone, quinidine (used for irregular heartbeats); dihydroergotamine, ergotamine (for migraines); ergonovine, methylergonovine (for stopping excessive bleeding); clorazepate, diazepam, estazolam, flurazepam, triazolam or oral midazolam (help to fall asleep); clozapine, pimozide, (psychiatric drugs); quetiapine (for schizophrenia, bipolar disorder and major depressive disorder); lurasidone (for depression); ranolazine (for chronic chest pain); pethidine, piroxicam, propoxyphene (used to relieve pain); cisapride (for stomach conditions); rifabutin (for certain infections); voriconazole (for fungal infections); simvastatin, lovastatin (for lowering blood cholesterol); alfuzosin (for enlarged prostate); fusidic acid (for bacterial infections); PDE5 inhibitos (sildenafil, avanafil); colchicine (for gout), products containing St John’s wort.

Do not have any kind of sex without using a latex or polyurethane condom.

Do not share needles or other things like toothbrushes or razors.

Do not take Norvir if you are pregnant or plan to have a baby. First, consult your healthcare provider.
